私は、ずっと思っていたのだった。地域医療に関する多くの書籍や言説を見ては訪れる隔靴掻痒の感覚がとても嫌だった。いや、そこではないのじゃないの、大切なのは。あなたの経験は二度と他の地では再現されない。まるでガラスの向こうの人形を見るようでこの上なく切ない。だって、抱きしめようがないから。優秀なコンテンツを綺麗に並べても、それがカタログスペックであることは知っている。涙は飾りであってはならない。
私は、だから思ったのだ。もう自分で海に出よう。多くが寝静まり、月はなく、星だけが目指すべき方向を示す夜に出よう。海図はある。Family Medicine と Community Medicine。私が記録すべき航路は日記のように記されるだろう。目指すのは地域医療を実践するために必要な、動的で柔軟な理論体系だ。コンテンツそのものではない。形式あるいは構造を提示したいのだ。本ノートで試みたいのは、地域医療の実践を構成する時間・身体・関係性・場・生活といった層を統合的に理解し、変化の中で回復へと導く動的な診療の構造を記述することである。本ノートでは、地域医療の実践を構成するいくつかの層（時間の中で変化する症状、身体としての病い、関係の中で起こる医療、地域という場、そして生活と医師という存在）を、ゆっくりと辿ってゆこうと思う。それらがどのように絡み合い、実践の形を生み、変えてゆくのかを見てゆきたい。
I had long felt it. Whenever I encountered books or discourses on community medicine, a vague sense of something missing would return. No, that is not it—the essential lies elsewhere. Your experience can never be reproduced in another place. It is unbearably sad, like seeing a figure behind glass, impossible to embrace. However beautifully excellent contents are arranged, I know they are catalog specifications. Tears must never be mere decoration.
So I decided: I will go out to sea myself. At night, when most are asleep, when there is no moon and only the stars indicate direction. There is a chart: Family Medicine and Community Medicine. The route I must record will be written like a diary. What I seek is a dynamic and flexible theoretical framework necessary for practicing community medicine—not the contents themselves, but the form, the structure. What this notebook seeks is to describe the dynamic structure of care that integrates layers such as time, the body, relationships, place, and lived context in community practice, and guides change toward recovery. In this notebook, I will slowly trace several layers that constitute the practice of community medicine—symptoms unfolding in time, illness as embodied experience, medicine arising in relationships, the community as place, and lived context with the physician as presence. I wish to see how they intertwine to generate and transform the shape of practice.
